SCHEDULE

For New Patients:

1. Call 971-318-0869 to schedule an appointment.

2. Patients will get an email link to join the Charm EHR portal and receive forms to fill out online when an appointment has been scheduled.

3. Please bring your insurance card to the appointment so we can make a copy for your records.

4. Make sure you have filled out your forms - intake, insurance verification, and consent forms prior to your visit.  Preferably 24 hrs before so it can be reviewed in advance.

5. When you arrive, text me at the above number and I will come meet you at the lower entrance or head into the building through the upper entrance and the front desk will direct you toward our waiting room.

6. Then please turn off your cellphone and begin to unwind from your day and we will be with you shortly!

IMPORTANT - There is a 24-hour cancellation policy. Please call if you have questions regarding this policy. There is a fee of $70 for a missed or late canceled appointment.
